JOB SUMMARY:
With guidance from the Director of Population Health Management, the Population Health Medical Assistant works collaboratively with the Care Management team and other healthcare professionals within the clinically integrated network (CIN) to provide care coordination across the health care continuum.  Works collaboratively with the Arkansas Health Network care transitions team to identify and coordinate hospital transitions for attributed populations.  Supports the administrative requirements within the CIN care transitions team to ensure safety, best practice and high quality standards of care are maintained.  In addition, the position will assist in identifying and closing gaps in care for attributed populations.



ESSENTIAL FUNCTIONS:



Assists in the identification of attributed populations needing care transitions support including obtaining admission, discharge and transfer data from acute facilities.
 

Assists with data entry and tracking in population health IT platform and ambulatory EMR.

Identifies and assists with providing post discharge support and closing gaps in care during the care transition process focusing on the following:


Scheduling of post discharge follow up when needed

Coordination with post-acute providers to ensure timely initiation of care

Telephonic follow up to low risk attributed members
 

Effective collaboration, communication and coordination among all responsible parties of an individual member’s multidisciplinary health care team striving to eliminate fragmentation, duplication or gaps in care.

Acts as a liaison between the provider and his/her patients coordinating care across multiple sites to close gaps in care.

Identifies opportunities for improvement (at individual, clinic and CIN levels) and actively works with healthcare and CIN team to correct or improve results.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
Education:  High school diploma or equivalent required.
 
Licensure/Certification:  Current/valid Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) certification required.
 
Experience:  1-2 years of experience as a certified medical assistant required.  Experience with population health concepts preferred.  
 
Skills:  Must have strong organizational (time management) skills, strong interpersonal skills and the ability to handle multiple priorities.  Must have computer skills to include navigating complex reporting software, Microsoft Office and the ability to learn new systems.  Must possess strong customer service skills to coordinate services.  Must have effective communication, telephonic and organizational skills.','We’re an equal opportunity employer.
